Job Title: RN Care ManagerJob ID: AB592745114Our Client is looking to hire a RN Care Manager. They need someone who meets the following qualifications: License/Certification/Experience:Current RN License in good standing in the state of practice required.Preferred Certified Care Manager (CCM) certification or commitment to completion within 1 year of hire.Minimum of 3 years’ experience in a clinical setting.Experience working in field-based role.Health Plan, Patient Centered Medical Home or CPC+ experience is preferred.Person-centered care plan preparation experience is required.Knowledge and Skills:Knowledge of Medicare and Medicaid care management requirements in accordance with CMS and MDHHS guidelines (or similar program for dual eligible beneficiaries).Knowledge of Healthcare Effectiveness Data Information Set (HEDIS) and engagement with members on care gap closure.Proficient in computer skills to include Microsoft Office Suite (Outlook, Excel, PowerPoint, Word).
